Our objective is to deliver optimal care to homebound patients through the provision of Medical Assistance and Services, including Annual Wellness check-ups, Monthly Follow-Ups, and Medical Management. As an entity certified by both Medicare and Medicaid, we extend our services to patients covered by these programs, encompassing Medicare, Medicaid, and Medicare/Medicaid recipients.

Our team comprises a Doctor and Nurse Practitioners who offer patients the flexibility of Tele-health or Face-to-Face In-Home consultations. Additionally, we facilitate referrals to Home Health Agencies and Specialists, as well as the procurement of Durable Medical Equipment (DME), laboratory tests, and radiology services as required. In instances where patients are medically unable to leave their residences, we diligently endeavor to arrange mobile services for laboratory tests, imaging, and, when necessary, consultations with medical specialists in the comfort of their homes.

We commit to resolving service requests within a timeframe of 24 to 48 hours. Maintaining close communication with Home Health Agencies, Rehabilitation Facilities, Nurses, Caregivers, and other Healthcare Professionals is integral to ensuring that the diverse needs of our patients are consistently addressed. Our administrative office operates Monday through Friday, from 9:00 AM to 5:00 PM Pacific Time.
